Enrollment Checklist

Here’s what you need to do before you enroll:

Review your plan and coverage options – Open Enrollment is your once-a-year opportunity to review your benefit choices and make changes for next year. You won't be able to make additional changes in 2027 unless you have a qualified status change.

  • Decide if you want to make changes - If you do not take action during Open Enrollment, you will have the same coverage as 2026, with the exception of the FSAs
  • Decide if you want to participate in any tax-saving spending accounts – you have to actively enroll to participate
    • Health and Dependent Care FSAs
    • Health Savings Account (you must be enrolled in the Anthem Consumer Choice HDHP, Anthem Basic HDHP or Kaiser High Deductible HMO – learn more about the HSA)
    • Decide if you want to change how you receive the $400 annual ($100/quarter) well-being dollars. Your 2026 quarterly funding election will carry forward — but Open Enrollment is your chance to change how these dollars are deposited. You cannot change this election midyear.
  • Decide if you want to elect supplemental life, AD&D and long-term disability – be sure to review your beneficiary for any life and AD&D coverage
  • Get your health screening by November 15 to save $260 on medical premiums for 2027
  • If you cover a spouse or domestic partner under your Cotality medical plan, review your working spouse surcharge status to be sure it's still up to date.
  • Review your tobacco/nicotine status to be sure it's still up to date.
  • Don't Miss Out - Open Enrollment, October 26 - November 6, 2026!

If you need help or have questions, you can contact the Cotality Benefits Center at (888) 259-3461 or contact Human Resources Employee Services by opening a Workday Help ticket.
