- Go to your doctor. Download the Physician Results Form from the Quest website, take it to your doctor for completion, then follow the instructions to upload or fax it to Quest.
- Visit a Quest Patient Service Center. Schedule an appointment on the Quest website by following the registration instructions below. Cotality will automatically be notified of your participation.
