Enrollment Checklist

Here’s what you need to do before you enroll: 

• Review your plan and coverage options – Be sure to make your elections within 31 days of becoming eligible for benefits.You won't be able to make additional changes this year unless you have a qualified status change.
• Decide if you want to participate in any tax-saving spending accounts – you have to actively enroll to participate
  • Health and Dependent Care FSAs 
  • Health Savings Account (you must be enrolled in the Consumer Choice Plan, Basic Plan or the Kaiser High Deductible HMO – learn more about the HSA) 
• Decide if you want to elect supplemental life, AD&D and long-term disability – be sure to designate your beneficiary
• If you are enrolling a spouse or domestic partner in medical coverage, complete your working spouse certification. If you do not certify that your spouse does not have coverage at his/her place of employment, you will pay a $100 per month working spouse surcharge
• Submit your tobacco/nicotine status.
