If you enroll your working spouse or domestic partner in a Cotality medical plan, and your spouse/domestic partner has medical coverage available at his/her employer, you will pay a monthly surcharge of $100.
When you enroll, be sure to complete the working spouse certification. If you do not complete the certification and you cover a spouse, you will automatically default into the working spouse surcharge.
Being tobacco and nicotine free is one of the single most important choices you can make to improve your health. Not only is using tobacco and nicotine damaging to your body, but it also costs us all millions of dollars each year. To help align our wellness program with our benefit plans, tobacco or nicotine users will pay a $600 annual surcharge when they enroll in a Cotality medical plan. You are required to certify during Annual Enrollment each year that you are tobacco/nicotine free or you will pay the surcharge.
Quitting Tobacco
If you need help quitting tobacco, you can build your own personalized Quit Plan. By completing a Quit Plan, you can also avoid the $600 annual tobacco surcharge. Once you’ve completed the program, submit a copy of your Quit Plan to Help – powered by Workday.
Who Is Considered a "Tobacco or Nicotine User"?
A tobacco or nicotine user is anyone who currently uses cigarettes, e-cigarettes, chew, cigars and any other tobacco-related materials. This includes regular tobacco usage and occasional tobacco use during social occasions.
