We've partnered with Forma to help administer your Well-being Account. This program is designed to allow flexibility in your health journey and determine what health means to you.
All benefit-eligible employees will receive the equivalent of 100 USD deposited into their account at the start of each quarter (January, April, July, October) for a total of 400 USD annually.
You’ll have two options of how you can receive your quarterly contributions:
- If you are enrolled in one of Cotality’s high-deductible health plans, you can receive your $100 quarterly allocation as a contribution to your Health Savings Account. Contributions to your HSA are tax free and will go into your account regardless of which high-deductible health plan you choose or your salary. You can use these HSA funds on eligible medical, dental, vision expenses at any time in the future. If you’d like this option, you must make an active election when you choose your benefits or during Open Enrollment on the Cotality Benefits Center.
- Alternatively, you can receive $100 quarterly into your Forma well-being account. If you are not enrolled in a Cotality high-deductible health plan or you do not actively choose to receive the HSA employer funding during Open Enrollment, you will receive the quarterly contribution to your Forma well-being account. You can accrue a balance of up to 5 quarters (up to $500) worth of funds for larger purchases. Your funds may be spent in the following two ways:
- Payroll Reimbursement - Submit receipts for qualified purchases, then receive reimbursements through your paycheck.
- Forma Store - Browse and purchase from Forma's online store without the need to submit receipts. Your available balance, spending history, and receipt submission will be available through the Forma online platform or mobile app.
