All benefit-eligible Cotality employees are eligible to participate in the program. You must be employed by Cotality when eligible expenses are incurred.
On the 1st of each quarter, employees will receive a quarterly benefit of the equivalent of 100 USD deposited into their Wellbeing Account (January, April, July, October). For new employees added after the program start date, the amount will be prorated based on the date added to Forma (first of the month following your date of hire). You may spend the allowance either through the Forma Store (some restrictions apply per country) or purchase an item/service out-of-pocket and be reimbursed via payroll (available for all countries).
No. If you are a benefit-eligible employee, you are automatically eligible in the program.
Reimbursement requests will be approved or rejected by Forma within 2-3 days of submission. You will receive an e-mail notification confirming whether or not the claim was approved. Claims that are approved will be reimbursed to you via payroll (local tax laws will apply).
IMPORTANT: please make sure that your item name, purchase date, and purchase amount are visible on the receipt. If this is not visible, your claim will be rejected. If available, please also have your name on the receipt.
All purchases made through the Forma store are automatically approved and adjudicated.
If the Forma store tab is not visible to you on the platform, then it is not available in your country.
Log in to your Forma account using the "Cotality Well-being Account" tile on the
SSO page or visit
joinforma.com. There you can submit your expenses from your Well-being Account, check your account balance, browse for eligible purchases, and track your transaction history.
In the U.S., reimbursement occurs on the second paycheck of the month following when your claim has been approved by Forma. Reimbursement differs from country to country, but generally occurs in the month following claim approval. Delays in providing documentation of expenses will delay reimbursement.
Local tax laws apply. You will be taxed on the amount you spend (either through reimbursement or Forma store purchases), and the amount will be included in your income for tax reporting and withholding purposes.
Balances can be accrued up to 5 quarters worth. Further quarterly funding will not occur if you are at that maximum balance.
Large pre-paid, eligible purchases that exceed the quarterly benefit amount may be submitted as one claim and can be set up as a recurring quarterly reimbursement. Please refer to this
article for additional information.
No, you will be unable to access the Forma portal while on leave for store purchases or to submit claims for reimbursement. You will continue to accrue funds each quarter and can submit claims for reimbursement when you return from leave.
Your eligibility to participate in the Well-being Account ends on the day of your employment termination and you will no longer be eligible to participate or submit reimbursement claims after this date. Any claims incurred on or before your termination date must be submitted and approved for reimbursement on or before your termination date.
If you have an active gym membership or subscription through the Forma store at the time you leave, the membership or subscription will stay active until the current billing cycle ends.
